A growing Healthcare Company is seeking a compassionate and experienced Nurse Practitioner with a strong clinical background and a commitment to delivering exceptional healthcare services.

The successful candidate will provide high-quality, patient-centered care to individuals across the lifespan, working collaboratively with physicians, nurses, and other healthcare professionals.

Responsibilities include:

• Conduct comprehensive health assessments, including medical history reviews, physical examinations, and diagnostic tests, to evaluate patients’ health status and identify health concerns.

• Provide primary care services, preventive screenings, and health promotion interventions to promote wellness and disease prevention in patients of all ages.

• Collaborate with interdisciplinary healthcare teams to coordinate patient care, referrals, and follow-up services, ensuring continuity of care and optimal patient outcomes.

• Provide patient education and counseling on disease management, medication adherence, lifestyle modifications, and preventive health measures to empower patients to take an active role in their healthcare.

Requirements:

• Master’s degree or doctoral degree in nursing (MSN or DNP) from an accredited nurse practitioner program.

• Current unrestricted license to practice as a nurse practitioner (NP) in the state of NY.
